The Metabolic Syndrome

By John D Zelem, MD
Do you find yourself having trouble losing weight even with exercise and, for all intents and purposes, watching what you eat? I suspect that you have been told to eat a low fat, high carbohydrate diet and you have done that. So why is there still a problem? You probably have thought of every possible reason for your lack of success. You may have thought that your thyroid gland is not working properly or your metabolism has slowed down: probably not. Possibly your hormone levels are off or maybe you need to step up your exercise program, or maybe you need one of these more dramatic diets to go on: not likely. That sounded like me and how I was thinking.
Normal Energy Production And Storage
You may or may not have been aware of something called “The Metabolic Syndrome.” What is it, you may ask? Allow me to give you a little background of basic information before I define this syndrome.
Our bodies need fuel for energy just like any machine. Sugar is that source. We need to get down to the cellular level where this energy production actually occurs. Glucose is the sugar utilized by the cell for the production of energy in the furnaces of our cells, which are called mitochondria. The entry of glucose into the cells is facilitated by the hormone insulin, which is produced in the pancreas. This hormone also will drive glucose into muscle and the liver for storage in a more complex form called glycogen. This will be used as a source of energy at other times such as periods of starvation and exercise. Any amounts over and above normal usage for immediate energy and storage in liver and muscle are sent to fat cells for greater storage of energy sources.
There is an opposing hormone, glucagon, which is also produced in the pancreas and is responsible for releasing fat for energy when stimulated by the intake of protein. The intake of carbohydrates and excessive levels of insulin suppress it.
Normally these hormones work in conjunction with each other maintaining a balanced situation of energy substrates utilization and storage. Situations such as dieting, starvation, exercise and the Metabolic Syndrome will alter this balance.
Glycemic Index
Whenever we ingest sugar or carbohydrates our bodies react with a rise in the blood sugar inducing the secretion of insulin to get the sugar into our cells and produce energy and store the excess in muscle, the liver, and eventually, fat. The Glycemic Index is a measurement of the rate of the rise of blood sugar following the ingestion of a particular test food relative to that of a standard food such as glucose. The measurement for glucose is 100. This index will quantitate the rate of secretion of insulin. Under 55 is generally considered to be a low-glycemic food and over 70 is high-glycemic.
Low GI = 55 or lessMedium GI = 56 - 69
High GI = 70 or more
Our diet today is mainly composed of over-processed carbohydrates in the form of our modern-day flour. This flour is the result of removing all of its complex components, leaving us with a pure, super-fine white powder that, when ingested, causes our blood sugar to rise rapidly to higher than normal levels. This leads to an exaggerated insulin response. This rapid rise in insulin will cause blood sugar levels to drop precipitously to relatively low levels, lower than normal, causing drowsiness, and fatigue. The subsequent rebound also results in a desire to eat again to restore blood sugar levels. Long term this becomes an uncontrollable craving for carbohydrates. This roller coaster effect occurring over and over, leads to carbohydrate addiction, carbohydrate craving, nighttime eating, insulin resistance, and the beginning of the Metabolic Syndrome. These high insulin levels also shut off the glucagon response and leave no way that fat can be utilized as a source of energy. This is when weight gain occurs and weight loss becomes almost impossible.
Eventually, this syndrome will cause inflammation and narrowing of the small arteries going to the muscles of your body causing them to constrict. This leads to a decreased ability of insulin to deliver glucose to the muscles for use and storage. Insulin levels will then increase significantly to try and accomplish this task but, eventually, most of the glucose will be diverted to fat cells causing what is called “insulin resistance.” Glucagon is totally shut off at this point.
Some of the other effects of the Metabolic Syndrome are high blood pressure, increased triglyceride and cholesterol levels, and decreased HDL, all leading to an increased risk of cardiovascular disease. This Syndrome has been said to occur in about 20-25% of the population. A significant portion of this group will go from insulin resistance to full blown Diabetes as the Metabolic Syndrome causes a “burn-out” of the insulin-producing cells of the pancreas. This is also one of the reasons why we are seeing a lot more obesity and diabetes, especially in our young. It is getting to epidemic proportions.
Low-Glycemic Response
There is hope! There is an answer. It is called a low glycemic diet. This must be considered a lifestyle change, not a temporary eating habit. Traditional diets do not work because they are designed to be an acute solution to a chronic problem. Also the faster one loses weight, generally, the faster it comes back. The chances of losing up to 10% of your total body weight and keeping it off for 5 years is the same as the 5 year survival rate for cancer of the lung, 5%. This is a very sobering statistic. You will find that it may, and should, take 18-24 months for you to achieve your end results. You should set short-term goals along the way. I have actually chosen to let my body decide what its end-point will be and take as long as it needs to get there since my program also involves exercising, which yours should too.
You need to understand that this syndrome is reversible with time while the alternative of not making this change is diabetes, which is never reversible. Remember, if you continue to do what you have always done, you will continue to get what you have always gotten.
When an individual eats a low-glycemic meal there is a totally different metabolic response. Eating foods such as fruits, vegetables, good carbs that do not cause a rapid rise of your blood sugar, good protein and fat is the answer. The “whites” are out: the white breads, the white potato, and the white rice. Even most whole wheat bread is not our friend. It cannot be enriched wheat flour; it must be stone ground.
These types of carbs will allow a slow rise of blood sugar, leading to a more modest release of insulin balanced with a proper release of glucagon. The insulin encourages the muscle to take up the sugar and the rest goes to the liver and fat. On the other hand, glucagon is also present in normal levels and will break down the fat, at about the same rate it is created, initially at a greater rate. The result of this equation is no weight gain or possible weight loss. Even if you have the Metabolic Syndrome, the start of a low glycemic lifestyle, not diet, may allow a reversal of all of the bad effects of this syndrome. Eventually, exercise must become a part of this healthy lifestyle.
Conclusions
So how do we summarize this complex bit of information and put it into a useful tool that is easy to follow. First of all, how badly do you want the results, the goals that you set? If the goal is important enough the work will follow and you will accomplish it. Remember that the Metabolic Syndrome is reversible with time, and Diabetes is never reversible.
Here are some general guidelines to follow. Low glycemic food will not raise your blood sugar as high and as rapidly as high glycemic foods will and, at the same time, will start to increase your body’s sensitivity to insulin. Low glycemic diets will help you to lose weight, help those who already have diabetes maintain better control of their blood sugar. Low glycemic foods will help you to stay full longer. Finally, high glycemic foods may replenish carbohydrate stores after exercise but low glycemic foods will improve physical endurance.
Foods that have a low glycemic index generally had a low glycemic load. Eliminate foods with both high glycemic indexes and high glycemic loads. Optimize your insulin levels by eating fruits and vegetables and whole grains. The fiber in these foods will both be healthy and release sugar into your bloodstream slowly.
I want to reiterate for you that I have changed my lifestyle to low-glycemic index, low glycemic load and have seen a significant change in myself. I am not hungry all the time anymore. I am free of carbohydrate addiction and cravings. I can actually sit in front of the TV at night or read a book and not have a high-glycemic snack next to me. I have not been doing it long enough to get off all of my meds but I see that in the near future and I recently was able to reduce my high blood pressure medication. Remember, getting healthy is a process and this is just one of the steps. It is not about what has happened to me, but I do know this can work for you.
I am not saying that this is the lifestyle change that you have to make, but if you see yourself in a similar situation, take a good look. Obviously you will need to discuss this with your doctor and see if it is a fit for you. You should also work out a program with your personal physician to regularly check your labs, weight, and blood pressure. Remember, if you continue doing what you are doing, you will continue to have what you have. If you want to make some changes in your life, you must make some changes in your life. I wish you success and good health.

Blood Sugar Metabolism - Let Natural Drugs be Your Controller!

By Darshana Vaishnav, M.D.
What is Diabetes?
Diabetes is a disorder of carbohydrate metabolism and refers to Diabetes (siphoning through) Mellitus (sweet). People with this disorder have high level of blood sugar known as hyperglycemia and lose sugar in their urine. Factors contributing to high blood sugar include reduced insulin production, decreased insulin usage, and increased glucose production by the liver. Diabetes is caused by a complex interaction of genetics, environmental factors and lifestyle choices.
There are two primary types of diabetes:
Type I, insulin dependent diabetes. People with this disorder do not produce enough insulin.
Type I diabetes is caused by autoimmune, genetic and environmental factors and even possibly by viruses. It develops in children and young adults. Symptoms include constant hunger, increased thirst and frequent urination, weight loss, extreme fatigue and blurred vision. If not treated with insulin, a person with type I diabetes can lapse into a life-threatening diabetic coma, also known as diabetic ketoacidosis.
Type II, sometimes called non-insulin dependent or insulin resistant diabetes. In type II diabetes, the body produces enough insulin but is not effective in transferring glucose from the blood to the cells.
About 90% to 95% of people with diabetes have type II diabetes. This form of diabetes may include symptoms similar to those of type I as well as frequent infections, and slow healing of wounds or sores. Some people have no symptoms. Type II diabetes is more common among the overweight, the elderly, Asians, Hispanics, African Americans and Pacific Islanders.

Gestational diabetes is yet another type of diabetes that occurs during pregnancy and these women are likely to develop diabetes later in life.
How to Manage Diabetes?
Type I diabetes always requires insulin injections in addition to diet and exercise.
Type II diabetes can be controlled with diet and exercise alone but may require medications in some cases. In addition, taking multivitamins, minerals, herbal supplements and natural drugs shown in the table below can significantly reduce complications. Keeping diabetes and other health conditions in mind, some websites may list all or some of these supplements in the categories such as Blood Sugar Metabolism, Vitamins, Diabetes, etc. It is important that the diabetes patients also continue to monitor their blood sugar level as recommended by their doctor.
Vitamins and Herbs and Their Effects on Diabetes
Bilberry extract: Helps prevent and reduce the severity of diabetic cataracts.
Bioflavonoids: Helps prevent vision loss. Helps prevent damage to vitamins C and E from diabetes.
Biotin: Biotin and Chromium work well together to help reduce blood sugar levels. With the help of chromium, biotin helps the pancreas work efficiently, helps insulin work better to lower blood sugar levels.
Chromium (Picolinate): Chromium and Biotin work well together to help reduce blood sugar levels. With the help of biotin, chromium helps insulin work better, helps the pancreas work efficiently, and lowers blood sugar levels.
Copper (Picolinate): Copper helps prevent diabetes-related damage to blood vessels and nerves, and lowers blood sugar levels by helping protect the cells in the pancreas that make insulin healthy.
Fenugreek seed extract: Helps liver and kidneys metabolize blood sugars more efficiently; helps lower blood sugars.
Folic Acid: In conjunction with B12, folic acid helps prevent strokes and loss of limbs due to diabetic complications.
Gymnema Sylvestre: Helps balance blood sugars and may help control weight.
Magnesium (Chelate): Helps insulin work more effectively and helps relieves neuropathic pain.
Manganese (Chelate): Helps prevent damage to nerves and blood vessels.
Selenium: Helps take blood sugar into the cells. Selenium also protects blood vessel and nerve from the damaged caused by elevated blood sugars. Selenium is also called, an "insulin mimic".
Vitamin B6 (Pyridoxine): With B12, and folic acid, B6 helps prevents heart attacks and nerve damage. Helps prevent diabetic blindness, helps keep vision clear and normal.
Vitamin B12: In conjunction with folic acid, Vitamin B12 helps prevent stroke and loss of limb due to diabetic complications. Also works with Vitamin B6.
Vitamin C (Ascorbic acid): In partnership with vitamin E, helps push sugar from the blood stream into the cells. Keeps blood vessels and kidneys healthy.
Vitamin E (Pyridoxine): Vitamin E together with B vitamin helps keep the pancreas healthy and helps prevent nerve damage. Helps blindness, and heart attacks.
Zinc (Pyridoxine): Helps blood sugar get into the cells and help insulin work effectively.

Keep Back Pain at Bay

By Dr.Savitha Suri
Introduction
Back pain is the most common complaint among adults under 45. Back pain brings their activities to a complete halt and intervenes in the progress of their career. Researchers say that it is really a challenge to a physician to find out the true cause of back pain.
Here is a quick take on the anatomy of the human back. Our body depends on the spinal column for structural stability and mobility. We twist, bend and flex with the help of spinal column. The spinal column is made up of 24 vertebrae, 7 in the neck (the cervical vertebrae), 12 in the thorax (the thoracic vertebrae), and 5 in the lower back (the lumbar vertebrae). The sacrum, located just below the lumbar vertebrae, consists of 5 fused vertebrae. The tailbone, or coccyx, with its 3 to 5 fused vertebrae, lies just beneath the sacrum. These vertebrae are placed one on another from skull to pelvis. Spongy discs are present in between vertebrae .The spinal column is tightly wrapped by muscles and ligaments.
The spinal cord is enclosed within vertebral canal which is formed by alignment of vertebrae.
Acharya sushruta describes anatomy of spinal column as follows.Prushte trimshat - The number of back bones are 30 (Cervical-7 , Thoracic – 12, Lumbar- 5, vertebrae fused in sacrum -5 , coccyx -1 ). While describing the joints in spinal column it has been said as “ Chaturvimshatihi prishta vamshe “. This means there are 24 joints in spinal column.
Maamsaanyatra nibaddhani siraabhihi snaayubhistatha. Asthinyaalambanam kritwaa The ligaments and tendons hold the bones and muscles together.
Causes for back ache
Bones and thighs are mainly occupied by Vata . Vata controls and directs the body movements.Back pain occurs when vata in this region gets aggravated due to unwise physical activities (mithya yoga of karma), exaggerated physical activities (Ati yoga of karma ) or less physical activities (hina yoga of karma).
1. Back ache usually arises from spasm of muscles that support our spine. The muscles supporting spine go into spasm when we lift things unwisely, bending over too sharply, sitting in a unsuitable chair, etc.
These type of movements are called as “Mithyaa yoga of karma or action ” in ayurveda.
2. Back pain can be triggered when we sneeze, climb into a car, or bend down to pick up the newspaper.
3. Osteoporosis, osteoarthritis, viral infection, bladder or kidney infection, gynecological problems in women, tumors and trauma may also cause back pain.
4. Lack of motion or hina yoga of karma and bad posture may affect us when we sit or stand for long periods of time. On the other hand repetitive motion or Ati yoga of karma like too much of bending, lifting or twisting also affects our back.
5. Other potential causes for back ache are:
• Overweight
• Wearing high heeled shoes
• Carrying heavy weight
The vitiated vata causes stiffness of joints, pain and limits the movements of back.
Solutions for back pain
1. Stay calm.
2. A complete bed rest for 24 - 48 hrs relieves back pain in most of the instances. Walk a little every few hours to keep the blood flowing and the muscles toned.
3. Applying vata balancing oil like ksheerabala oil on affected region and Naadi sweda (mild steam bath on affected area ) of affected region relives the pain quickly.
See your doctor immediately if you have any of the following symptoms:
1. If you are above 50 and sudden onset of severe pain or pain localized to spine .
2. Pain due to recent fall or trauma.
3. Pain in hip and leg.
4. Back pain unrelated to posture.
5. Bowel or bladder control problems.
6. Numbness in the groin or rectal area.
7. Extreme weakness in legs.
8. Unsteadiness while walking
9. Fever or weight loss accompanying the pain.
10. Pain that does not lessen over time even after rest
Prevention of back pain
1. Keep the muscles well conditioned by improving sitting, standing, stretching postures. Well-conditioned muscles are less susceptible to injury.
2. Never bend at the waist or stoop to pick up an object. Instead, lower the body to the level of the object by bending the knees. Grasp the item, hold it close to the body, and raise yourself with your legs while keeping your back straight.
3. Do not lift very heavy objects.
4. Whenever possible, prop yourself up.
5. Lean against a wall or pillar while waiting for a bus or standing in line at the bank.
6. Keep footstools around the house. Alternatively prop up one leg on stool while doing standing chores.
7. Sleep on a firm mattress and find a comfortable position.
8. Wear low-heeled shoes.
9. Standing straight will do wonders for your back.
10. Sit on a comfortable chair that supports the curve of lower back.
11. While sitting for long hours rest feet on a low stool so that knees are above hips.
12. Do not sit or stand in one position for a very long time. Get up periodically and walk for a while.
13. Avoid activities which hurt back.
14. Do not sit in same position for long hours. It may prove hazardous to back. Take break in between and move around.
15. Avoid becoming overweight. Overweight increases the risk of back pain.
Tips for software professionals to avoid back pain:
1. Select a correct chair, with a relatively straight back and good lower back support. If the chair doesn't offer lower back support, keep a rolled up towel or cushion to support the back.
2. Correctly position the chair, keyboard, and monitor. Feet should be on the floor, with knees at hip level; forearms should form a right angle with upper arms; and the computer terminal should be at eye level.
3. Adopt a relaxed, upright working posture and allow the chair to support.
4. Avoid slouching forward or leaning back too far.
5. When you are at work stand up, walk around, stretch forward, backward, and side to side once in 30 minutes.
6. Increased stress and strain enhances the chances of back pain. Hence remember to relax! Yoga and meditation are best techniques to relax
7. Eat healthy food which is rich in fiber. Avoid sweets, oil, soft drinks and overeating to reduce the chances of weight gain.
8. Sex should be avoided during acute stage of back pain as it exaggerates the condition.
Ayurvedic Home remedy for backacheRemedy
